Although patients recovering from an illness or injury may be well enough to go home, many still require additional care while they are recovering in order to achieve their highest level of independence.
an agency of Lutheran SeniorLife, offers comprehensive skilled home health services and private duty home care to individuals in Allegheny, Beaver and Butler Counties. The BeHome PartnersTM team of professionals includes:
- Registered nurses (RNs, LPNs)
- Registered dietician
- Home Health Aides
- Physical, Occupational and Speech Therapists
- Social Workers
Upon physician referral, our team develops an individualized, comprehensive health care plan for a broad range of conditions and needs. Interventions may include:
- Patient and caregiver education regarding medication management, safety and nutrition
- Rehabilitation for mobility, self-care and equipment assessment
- Wound and ostomy care
- Intravenous therapy
- Telehealth monitoring
Private duty home care services are also available to provide personal care and support to help individuals maintain their level of independence in their place of residence. These services do not require physician referral and are usually not covered by insurance.
BeHome PartnersTM accepts reimbursement from Medicare.

Skilled Home Health Services
Initiated through a physician's order and reimbursable by most insurance companies, skilled home health care services are tailored to the individual and provided in the comfort of person's home. Typical services received may include:
- Physical therapy
- Occupational therapy
- Speech therapy
- Skilled nursing for wound management, IVs or medication education
- Nutritional counseling
- Social service intervention
- Home health aide services included in the plan of care
Private Duty Home Care Services
Private Duty Services is a program of BeHome Partners and is designed to provide non-medical routine personal care and support to help individuals maintain their independence. Private duty home care services are tailored to the individual and are provided in the comfort of person's home. Often it is the availability of these services that is the key to helping residents remain in their own homes.
Personal Care - Assistance with all aspects of self care activities including:
- Bathing
- Mobility
- Feeding
- Toileting
Support Services - Conversation and companionship for safety and metal stimulation, as well as help with household tasks, such as:
- Errands
- Medication reminders
- Light housekeeping
- Laundry and linen washing
- Errand services
- and more
In-Home Memory Support
BeHome PartnersTM has adapted to meet the changing needs of seniors by creating a new program, “In Home Memory Support,” to help them live a rich, Abundant Life®while remaining in their own home.
Caregivers are carefully selected for their capacity for empathy and compassion. Each is trained in addressing all aspects of the aging process, including Alzheimer’s disease and other conditions that affect memory and cognition.
During their in-home visits, our caregivers follow an individualized treatment plan, which could include (but is not limited to) providing companionship, engaging in mentally stimulating activities, and providing personal care for basic activities of daily living. They are also able to develop strategies to diffuse stressful situations, manage challenging behaviors that may arise as a result of the patient’s dementia, and identify situations that may become problematic.
As the patient’s needs are identified, BeHome PartnersTMcan also provide skilled care such as nursing; physical, occupational or speech therapy; and nutrition counseling.
Services can be provided on a short or long term basis. The application process is simple, so services can be initiated quickly to provide relief to family caregivers.

The Visiting Nurses Association family of home health services has been serving communities throughout Butler, Armstrong, Mercer, northwest Westmoreland and portions of surrounding counties for over 45 years.
Lutheran SeniorLife has formed a affiliation with VNA to bring an additional option for skilled nursing and other therapeutic services to those who need some assistance in order to live safely and independently in their own homes.
